My calipers are on front portion of my rotors (349mm). Do the bolts (pictured on left side) with larger head/washer, attach to bottom guide pins (the ones with rubber boot)?

Yes—the two bolts with the larger captive washer/flanged head install at the lower guide pins with the rubber boots, one per caliper. The two smaller bolts install at the upper guide pins; the caliper being mounted ahead of the rotor does not change the upper/lower arrangement.
